Service Request Form

Looking to send in your equipment for service? There are three recommended ways to arrange you equipment coming to Kingsway:
1) Include your own paperwork with shipment (such as Purchase Order) which includes all relevant information such as Company/Customer Name, Address, Contact Info (Phone/Email), Shipping Info, and PO # (if applicable).
2) Fill in our Service Request Form (Click HERE) and include with shipment to us to ensure all information is provided to our team.
3) Complete the online RMA form (See Submit RMA in footer on website) and fill in all necessary information. Our team will receive the request digitally and file while we wait to receive your tools. NOTE that no RMA # is generated and you can proceed with shipping the equipment to us immediately.
